If you're here, hopefully it means you've found one of my mix CDs which I left carelessly lying around someplace. If this is the case, please fill out the form below so I know what you found and where you found it. Fill in as many or as few of the blanks as you want. Be as specific or as general in you answers as you like. Your e-mail WILL NOT be made public, merely used by me to reply to you if one is warranted. Keep in mind that what you fill out below may be posted on my website (with the exception of e-mail) on my Found CDs list.
